We are currently hiring one full-time Clinical Assessor to be based at Parkhouse in Gedling.

About Urgent Community Response (UCR)

UCR provides holistic, person-centred care aimed at preventing unnecessary hospital admissions.

The service helps individuals increase independence, improve health outcomes, and reduce:

  • Hospital stays
  • Long-term care needs
  • Reliance on social care

We work closely with:

  • EMAS
  • Primary care
  • Social care and community health partners
  • Care providers, lifeline services, and others

Care is delivered through timely assessments, interventions, rehabilitation, and management of long-term and life-limiting conditions — all while collaborating across the healthcare system.

Main Duties of the Role

  • UCR is a national NHS England service reviewing patients within 2 hours at their usual residence.
  • You’ll be part of a collaborative, compassionate team that values support, learning, and clinical excellence.

    • Self-care programs
    • Partnership with carers, health and social care services

🕒 The service operates 7 days a week, from 08:00 – 22:00 hrs.

Job Description: Clinical Assessor

We’re offering an exciting opportunity for one new Clinical Assessor to join the UCR team, based at Parkhouse.

As a Clinical Assessor, you will:

  • Respond to patients in crisis and deliver impactful, problem-solving care.
  • Work from three main bases:
    • Cotgrave Hub (Rushcliffe)
    • Parkhouse (Gedling)
    • Stapleford Care Centre (Broxtowe)

This role is ideal for:

  • Nurses, physiotherapists, occupational therapists, and other AHPs looking to work in a forward-thinking, compassionate service.

Your responsibilities will include:

  • Supporting discharge and hospital avoidance across Rushcliffe, Broxtowe, and Gedling.
  • Performing top-to-toe holistic assessments, using tools like:
    • Stethoscope
    • Percussion and palpation techniques
  • Managing a caseload with moderate to complex needs.
  • Coordinating with:
    • GPs
    • Community nursing and therapy teams
    • NHS 111
    • Local Authorities

You will:

  • Design recovery and reablement care plans.
  • Promote independence, well-being, and self-care.
  • Support system flow in line with 2-hour/2-day response targets.

What You’ll Need

  • Registered Nurse (RGN or RN Adult) or AHP (HCPC registered)
  • Relevant clinical experience & commitment to continued development
  • Full UK driving licence and access to a vehicle (reasonable adjustments available for disabled candidates)

  • £26.40 (Standard) / £54.40 (Enhanced)

Contractual Requirements

  • Full UK driving licence & business-use vehicle

Qualifications

  • RGN/AHP with NMC or HCPC registration
  • Degree or equivalent in a health-related subject
  • CPD and leadership capabilities
  • (Desirable) Mentoring/leadership qualifications

Knowledge & Skills

  • Knowledge of elderly and chronic conditions (e.g. frailty, COPD, heart failure)
  • Holistic physical assessments, NEWS2, SBAR
  • Strong clinical reasoning, communication, and interpersonal skills
  • (Desirable) Familiarity with stethoscope, UCR guidance, equipment prescription
