Job Summary

The Flow Support Worker will play a prominent role in ensuring the delivery of flow within LSCFT Inpatients. This will be achieved through developing excellent internal Communication pathways, the utilisation of the internal escalation process and ensuring the multi-disciplinary team adhere to the Department’s internal professional Standards.

The post holder will ensure the timely flow of patients through the area to help Identify available capacity and assist to place patients appropriately, supporting the Integrated Discharge Team Practitioner and Flow Lead working within agreed procedures and protocols.

Main duties of the job

The Integrated Discharge Team is seeking a Patient Flow Support Worker to work alongside the existing practitioners and ward staff to support flow and capacity and ensure admissions in hospital are therapeutic and discharges are safe and effective.

The successful candidate will work with the Multi-disciplinary teams in locality to assist in the facilitation of timely, safe and appropriate hospital discharge arrangements. They will also build relationships with external parties to ensure a discharges are safe whilst maximising their independence and autonomy.

We are looking for individuals who have experience of direct care delivery, ideally with understanding of community and inpatient processes. The applicant should possess excellent communication skills and have the ability to work pro-actively within an MDT setting. Time management is essential and prioritisation of duties.

Qualifications

Essential

  • Educated to A-level, or NVQ Level 4 in Business Administration/Customer Care/ HNC in Business or acquired relevant equivalent experience (E)

Skills & Knowledge

Essential

  • Ability to communicate effectively with people at all levels
  • Ability to use own judgment to resolve problems and queries competently, including those of a sensitive and confidential nature
  •  Ability to plan, prioritise and delegate workload
  • Ability to initiate & co-ordinate change to own working practices
  • Good knowledge of speciality specific patient pathways

Experience

Essential

  • Experience of using full range of IT systems and patient data systems
  • Experience of using Microsoft Office, including Word and Excel
  • Experience of scheduling outpatient appointments and/or admissions
  • Experience of working in a multi-disciplinary team

Communication

Essential

  • Ability to provide and receive, convey and present sensitive and/or contentious information to large groups, responding openly to questions to ensure full understanding and engagement.
  • Ability to communicate with clinical, academic and all levels of staff effectively.
  • Adept at conflict resolution
