To assist the intermediate care bed base staff in the provision of nursing care and support to patients and families in the bed base setting.
Please note this post will be permanent night shifts.
Main duties of the job
- To work as part of an integrated Community intermediate care Service in providing specific care to a patient in the bed base setting as written in the care plan.
- To provide personal care and support to patients in the bed base setting.
- To work alone and unsupervised, demonstrating the ability to work autonomously.
- To be responsible for the ordering of clinical supplies and stationery for the Community Nursing Team.
- To work in accordance with Trust Policies and Procedures.
- To maintain high standards of personal hygiene adhering to Health and Safety Guidelines and Infection Control Guidelines within the trust.
- To respect confidential information during the course of your work and refrain from discussing such information without the consent of the patient/client or a person entitled to act on his/her behalf, except where disclosure is required by law or by the Order of a Court or is necessary in the public interest.

Job responsibilities
Duties and Responsibilities:
- To monitor skin integrity of patients at risk of pressure ulcers formation and provide basic preventative information.
- To provide full personal care to patients.
- To provide health screening for patients e.g. Monitoring of blood glucose levels, phlebotomy, blood pressure recording and urine testing.
- To provide clinical intervention for care of patients as directed by Registered Nurses, following training and assessment of competencies e.g. administration of insulin, basic wound care and bladder irrigation.
- To check equipment on a regular basis and ensure it is safe to use.
- To attend mandatory training in accordance with the Trust Policy.
- To identify areas for development within a Personal Development Plan in collaboration with team leader/Sister/charge nurse.
- To report any Health and Safety or Infection Control issues.
- To assist in clinical practice as expected up to NVQ Level III or equivalent.
Person Specification
Qualifications
- GCE/GCSEs Grade A*- C/level 9- 4 in Maths and English or Equivalent or Willingness to undertake Level 2 Functional Skills Qualifications in Maths and English.
- Care certificate.
Experience
- Demonstrable experience of working in a healthcare setting.
- Community healthcare experience.
Skills and Attributes
- Excellent communication skills verbal and written.
- Wound care and pressure ulcer knowledge/experience.
- Ability to provide personal care to patients in the terminal phase of life.
- Ability to undertake clinical intervention for care of patients as directed by Registered Nurses.
- Ability to work well within a team.
- Evidence of time management skills and ability to prioritise.
- Use of SystmOne/EMIS.

- The ability to travel independently across the Trust.
- The ability to work flexibly in accordance with service needs.
