Job Summary
An opportunity has arisen for an Early Diagnosis Support Worker, supporting patients referred to the Breast Service.
The successful post holder will work with both clinical and operational leads to provide support within the department to meet key competencies such as delivery of the 62 day cancer access standard. You will require excellent communication skills which should include communicating clearly, sensitively and effectively with patients, families and professionals. You should be able to signpost patients to the most appropriate services and be enthusiastic to apply acquired knowledge and skills.
We are looking for someone with high energy levels, a positive ‘can do’ attitude that can work with a large range of people to deliver efficiency, productivity and change as required.
Main duties of the job
- Undertaking less complex keyworker activity.
- Facilitating efficient transfer of patients between providers to assist in the delivery of the 62 day cancer access standard, for multiple trust pathways.
- Liaising with patients on diagnostic pathways to facilitate timely access to services (e.g. coordinating appointments with patients; organising referrals).
- Following basic assessment provide advice and support within clinics, support remote monitoring of surveillance clinics to free up consultant/nurse led clinic time and follow up.
- Support delivery of primary and secondary prevention for patients and their relatives/ carers; supporting them around’ ‘teachable moments’ and Making Every Contact Count.
- Document and monitor all aspects of care coordination and service delivery, supporting data collection and record keeping for audit.

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust is a busy district general hospital and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.

Qualification
Essential
- Good General Education with GCSE English and Mathematics
- ECDL or equivalent experience
- Level 3 Health or social care qualification (e.g. NVQ or similar)
Desirable
- Level 4 Health or social care qualification (e.g. foundation degree)
- Evidence of continued role development
- Basic counselling
Knowledge and Experience
Essential
- Relevant Health or Social Care experience
- Advanced knowledge of medical terminology.
- Understanding of person-centred care.
- Experience of coaching/teaching patients and carers.
- Efficient in the use of Mirorosft Word, Excel, Powerpoint, Outlook and internet.
Desirable
- Previous medical secretarial experience
- Experience co-ordinating a patient workload.
- Knowledge of relevant cancer treatments, interventions and terminology
