The successful post holders will be expected to provide support and assistance to professionals in the implementation of care plans for individual patients.

Main duties of the job

Undertake a range of delegated clinical care duties deliver to deliver agreed plans of care.Provide excellent customer care skills to ensure patients come first.To ensure patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.

Recognise and avoid situations that may be detrimental to the health and well being of the individuals.

To act as an advocate for patients within the ward / department, to ensure a patient orientated approach to the delivery of care.

Provides support and assistance to professionals in the implementation of plans of care for individual patients.

Assist with fluid and dietary needs of patients including completion of menu cards and recording intake and output of fluids and solids.

Job Purpose:

Under supervision the post holder is responsible for carrying out assigned tasks as part of the clinical team in support of registered nurses, and to undertake some aspects of care provision under supervision. Ensuring the safety and well being of service users, always working in a manner that promotes dignity and human rights, through the adoption of persons centred care principles.

Clinical Duties

As directed, to collect specimens from patients and to record routine observations of weight, blood pressure, temperature, pulse, respiratory rate. Report and abnormalities and concerns to a Registered Nurse.

Recognise when patients are in pain and discomfort and report to the nurse in charge.

Provide reassurance, support to patients, carers and family.

Ensure own actions help to maintain quality.

To respond to patient requests for non-clinical information to improve communication.

Establish and maintain communication with people on routine and operational matters.

To act always in a manner which, promotes positive working relationships and maintains the professional image of the Trust.

Understand and adhere to all Trust policies, guidelines and procedures. In particular, Health and Safety, Fire Safety, Waste Management and Resuscitation policies to maintain a safe working environment for patients and staff.

Person Specification

Qualifications

Essential

  • Evidence of a general education e.g. GCSE’s or equivalent
  • NVQ II in Healthcare / QCF Diploma in Health Level 2 or equivalent qualification in the area of speciality (e.g. Operating Theatre Practice for posts in theatre) or an equivalent level of working experience within a similar role

Knowledge

Essential

  • Knowledge and experience of taking clinical observations
  • Knowledge of hospital systems
  • Knowledge of Manual handling of patients, using lifting aids

Experience

Essential

  • Some experience of working as a healthcare assistant in a hospital or registered care setting
  • Customer care experience
  • Commitment to working in partnership with patients / carers
  • Care experience within the last 3 years
