Job Details:
Salary:£21.73k – £23.18k per annum
We have an exciting opportunity for an enthusiastic Band 3 HCA/support worker to join the diabetes team to support inpatient and outpatient diabetes services.
Based on the Diabetes Education Centre, it is 12 months of fixed-term / secondment to cover maternity leave and is 25 hours per week. This could be split equally Monday – Friday (5 hours per day), or 4 x 6.25-hour shifts to include Monday, Wednesday, Thursday and Friday.
Main duties of the job
Maintain patient & staff confidentiality at all times
Arrange blood tests & obtain results.
Communicate all relevant information concerning the patient/relative/carer needs to the appropriate member of the team.
Triage telephone enquiries received.
Provide advice & support regarding diabetes literature & the use of insulin pens & blood glucose meters.
Triage diabetes centre “drop ins”.
Provide support to the diabetes foot clinic to cover annual leave.
Participate in multi-disciplinary diabetes meetings where appropriate.
Support the team to ensure that the clinical/educational care of patients is effective & complies with the requirements of the trust’s clinical governance framework.
Participate & contribute to effective communication within the diabetes education centre, inpatient service, outpatient staff, community services, other health professionals & outside agencies.
Participate in the development & provide written information to ensure easy access to advice & information for patients with diabetes their support mechanisms & health care professionals.
Participate in requests for information & support the diabetes inpatient team to maintain detailed referral records, nursing notes, & up to date statistics of the service.
Provide relevant reports on request.
Maintenance to strict record keeping is paramount.
Answer the telephone in a polite, professional manner & triage calls, ensuring that any advice is within your scope of practice.

Qualification & Training
Essential criteria
- Educated to GCSE standard or equivalent, to include maths and English at grades A-C. Computer skills including email and Microsoft Office applications. Use of ICE
Desirable criteria
- Use of Medway, EDIS, EMAS, Cito. Working knowledge of insulin delivery devices and blood glucose meters. Qualification in IT skills
Experience
Essential criteria
- Experience working in a support worker role. Experience of Multidisciplinary/team working. Ability to communicate at all levels.
Desirable criteria
- Experience in working with patients with diabetes
Knowledge & Skills
Essential criteria
- Excellent written and verbal communication skills. Time management skills. Administration/Coordination skills. Understanding of the importance of confidentiality.
